Take away the drugs, the high libido, mafia connections (and probably demonic heritage?), Bach would probably be just one of the ordinary doctors at the Razielim district. With some tragic background yes, but you know. Just one of the many.
He wouldn't join Team Acheron, who knows what would happen to Mutio in that alley, or Inuki, and I doubt he'd ever speak to Leo.
Maybe he could become a father and have a family.
Just an ordinary doctor.
But that doesn't mean he could not have all the other possibilities coming at him.
But still

It's the side the real Bach hates about himself.
He sees it as a weakness.
He prefers his fanulous self.
